
2012 Charles B. Jenkins Legacy Award
ONLINE NOMINATION FORM
The Charles B. Jenkins Legacy award is presented annually by the Employment For Seniors Board of Trustees, in order to recognize individuals and organizations whose outstanding and dedicated service and leadership have had a significant impact on the quality of life for seniors in Central Ohio. Charles “Chuck” Jenkins exemplifies this spirit of outstanding service. This award is dedicated to honor Chuck, and those like him, who work tirelessly to advocate for seniors. The Legacy Award Program is held in May of each year during Older Americans Month.
Criteria for nomination:
Nominee must be a Central Ohio resident or organization.
Nominations can be made by an individual or an organization.
The nomination should explain in detail the achievement and background of the nominee and the reasons for the nomination. The nomination should include back-up documentation (e.g., individual resumes or organization materials)
Each nomination requires a separate nomination form.
Completing the form:
Nominations can be provided via the on-line nomination form here or sending this form as an attachment via email to carol@employseniors.org. Nominations can also be sent via postal mail. Nomination forms must include complete contact information for nominee and nominator.
Please provide comprehensive, quantitative information that demonstrates the individual’s or organization’s involvement, achievement, and leadership accomplishments.
Additional sheets may be included. It is recommended that you provide EFS with a resume for an individual or printed materials for an organization as part of your nomination package. See the information at the bottom of this page for instructions on how to submit additional information.
Maximum of two letters of recommendation in support of the nominee.
Process:
An Employment For Seniors Board of Trustees selection committee will assess all nominations in strict confidence. The decision will be based on activities performed, organizations served, time given, overall impact on others or the community, and other relevant information.
